Who qualifies for home healthcare?
You may qualify for home health care if you:
require skilled nursing or rehabilitative services that are intermittent or periodic
require a considerable effort to leave home unassisted
are under the care of a licensed physician
Arranging for home healthcare
Saint Francis Healthcare works on a referral basis. Although referrals typically originate from physicians, your local hospitals, doctor’s clinics, board and care facilities, skilled nursing facilities, patient’s family and/or interested persons or agencies can also contact us directly.
A nurse or a representative of Saint Francis Healthcare receives the referral information. Then, our patients are signed up for the various services in accordance with the physician’s order/s. A licensed professional from Saint Francis Healthcare will be sent to conduct an initial evaluation of the patient within 24 to 48 hours, unless specific care is required on the same day of the referral.
